Steps to getting involved at Calvary Chapel Tucson :

1. Review ministry opportunities page.
    
2. Contact the staff overseer or volunteer contact about available opportunities and specific requirements that may be needed to serve in the ministry.
   
3. Review The Servant’s Requirements page. Determine what level of requirements are required for the particular ministry in which you want to serve.
   
4. Review our Statement of Faith for Calvary Tucson. If there is anything that you do not agree with, please make an appointment to speak to the staff overseer or pastor.
   
5. If Level I or Level II requirements are needed, complete only section 1. If Level III requirements are needed, complete all of section 1 and section 2.
   
6. Attach a recent and clear photograph to the application form (preferably not a driver’s license.) This is required for all levels. (Photos can also be taken at the information table.)
   
7. For your convenience, you may download the Ministry Application and return the completed questionnaire to the information tables located in the sanctuary foyers.

If you are filling out this form to serve with children, youth, or any other ministry involving minors, you will be asked to sign a release form for a criminal background check.  (This form will be given at the interview.)
